She didn't come from a well known family, she isn't a heiress. Madonna came from a regular family and created everything that she has today. I believe if I was her, I would've aged a bit by 49 as well. Her hands are the result on being older and the fact that she works out nonstop. This takes the layers of fat off and thus shows veins. Your 49 year old parents probably don't work out half as much as she does either, and probably still have fat layers on there hands.
