Those clubbed fingernails look like he's got pulmonary fibrosis. That's one of the main symptoms. Unfortunately, there's no cure yet, only treatment of symptoms (usually long term steroids). He should get that checked out.

Poor Mickey! his fingers are termed "clubbed fingers" and it is caused by chronic obstructive pulmonary disease also known as COPD and emphysema. the frothy sputum or spit is a result of the emphysema. I would definitely recommend that he stop smoking. it takes years of emphysema to get clubbed fingers. he really needs to lay off the cigs if he wants to live much longer.

Anyway his finger nails are like that because he did too much HGH (human growth hormone) which he used for both The Wrestler and IronMan. That white crap in his mouth happens to many people and I believe I have seen pictures of Perez with the same symptoms. It is just dry mouth, his salivary glands are not working normally, and sometimes it is just a dental problem. And maybe Mickey didn't grow old gracefully, and has had some dr hack mess around with his face, but at least we can all remember how amazing looking he was, remember 9 1/2 weeks, Can Perez ever say he was ever good looking, NOPE.
