Has anyone ever wondered what sorts of things might have driven Miley to do drugs in the first place (if it's not just experimentation)? Maybe it's the people that call her a slut, a whore, and have been since she was only 16. Maybe it's the people like Perez that think that it's okay to post pictures of them flashing their crotch when they're only 17?
At the end of the day, she is still a child, and no amount of media savvy is going to stop things like that effecting her.
