Of course that picture is retouched, fuck the retouch team probably spent a week retouching her manly body. She isn't feminine nor pretty. Been a Madonna fan for a VERY long time, the photo is definitely touched up, mostly in regards to her muscle tone (her arms mostly) and FOR SURE her hands, they have been a age giveaway for a LONG time, even when she was much younger they were not overly attractive.
That said, her weight, face, neck and body all appear very much hers and untouched. I think the reason her legs look this great is more due to the angle vs retouching.
This really is one of her best photo shoots in some time, the pictures look great.

I believe there was minimal touching up. Meisel is a brilliant photographer who knows his lighting. That already takes away half the blemishes and bags. Add to the fact that black and white photography is verrry forgiving on wrinkles and lines and also the fact that Madonna looks amazing for 49…and there you have it! I don't think they're lying…I think the photoshop work wasn't any more than what they do to other celebrities! Go Madonna!

If you look close enough you can see traces of the retouching. The higher arm, the darker strip of color next to her body from the torso to the leg.
