If you cover up her cheeks in the picture then she looks like the Madonna we all used to know. Whatever she's had done these create two sweeping lines across her face which makes her look sunken and gaunt ans as if she's constantly sucking her cheeks in.
I heard it's collagen, which shrinks over time, so when this happens, let's hope she doesn't return to the surgeon's office. Everything else on her face actually looks fine, but this procedure has sort of thrown everything out.

What she means is that the level of physical extertion she puts on for her performances does age you a bit - like when you run a marathan. You can see it in some professional athletes - they look much older than their age. For example Bret Favre he is 38, but looks 50.
