First, I've seen better quality of this particular picture and it looks pretty real to me.
Second, alot of reports are saying that she took several pictures of herself in the buff..it's probable that she could have taken off the chain around her waist in this particular shot and you can always set your phone to a timer to take a picture. And the comment about her "not having a belly ring" in this picture - obviously she could have taken it out!
