What she is saying, as some have said, is that she IS a thin woman and if she is twice as big as the other ones, then what does that say about how thin THEY are! She is saying that there is an unhealthly atmosphere about weight in Hollywood and maybe it's not a good place for her to live.
Does she live there? She means she should only be there to audition but not spend much time there because it's a warped environment. Many people say that.

Yes, Keira is very skinny but if you actually read what she's saying….at 5'7 or 5'8, she WOULD be seen by directors as being twice the size of all the Eva Longoria and Reese Witherspoon types who are barely five feet tall…and directors probably tell her this to her face too…what kind of body image would that give you? Being more than half a foot taller than most actresses she's competing with would make it near impossible for her to be as tiny as them…shoulders, hips, etc. will all be larger because she is taller. Look people, Ms. Knightly is not talking about the real world, but the Hollywood casting world. She has the classic English spoon shape i.e. wider hips, heavier thighs yet incredibly slim shoulders.
