After nearly four weeks, Philip Seymour Hoffman's cause of death has finally been confirmed.
It's even worse than expected.
The NYC medical examiner's office lists the cause of death as "acute mixed drug intoxication."
Philip's heroin addiction was well-known, but the coroner also found cocaine, benzodiazepines (an anti-anxiety medication), and amphetamines in his system.
The mix of heroin and cocaine, commonly referred to as a speedball, can be incredibly dangerous. John Belushi and Chris Farley also died with both drugs in their systems.
This lethal cocktail is officially what took Philip from us — and we all know he is still greatly missed.

Rolling Stone is no stranger to controversy when it comes to their covers.
Justin Bieber has gotten Stoned this month- and it's not for his music.
The cover story is a breakdown of all the Biebs' latest "Bad Boy" antics, AKA DUI arrest, drug searches, and vandalism egg-usations.
Is the music mag just going for shock value at this point? Or are Justin's turbulent times an important part of music journalism?
What do U think of Justin's cover?

David Bar Katz was one of Philip Seymour Hoffman’s best friends. The playwright and the late actor shared a love for the New York theater, were both members of the LAByrinth Company, and worked together on several stage productions.
Then on February 2nd, David entered Philip's West Village apartment to find his friend dead of an apparent drug overdose.
That devastating discovery was bad enough but soon after PSH's tragic passing shocked the world, The National Enquirer printed an outrageous story about David and Philip being secret lovers and that they had often freebased cocaine together! The rag mag even suggested a struggle with his sexuality was what lead Philip back to the heroin that eventually killed him!
After the false story, David, who is married with four children, declared he would sue the tabloid for libel.
He stated why the bogus story upset him so much, explaining:
Philip Seymour Hoffman's death was a blow to all film fans, but it hit his loved ones much harder.
Amy Adams' episode of Inside The Actors Studio was filmed two weeks ago, so we had already heard how emotional talking about Philip was for her.
But it didn't prepare us for how much watching Amy break down would break our hearts.
Check out the video (above) to see a true friend in mourning give a beautiful tribute to one of the greatest actors of all time.
The will of Philip Seymour Hoffman is revealing just how important it was to the late actor that his kids NOT be raised in EL Lay!
Written in 2004, back when Philip’s son Cooper was his only child, one of his final wishes was that his child be raised in Manhattan, in order to be surrounded by the culture, arts, and architecture that only New York can provide.
If NYC proved to be impossible however, the document also states that growing up in Chicago or San Francisco would also be suitable.
And even if he couldn’t live in these cities, Hoffman wanted his son to visit them at least TWICE a year!
Oddly enough, the City of Angels is not mentioned in the will, despite Philip being a famous movie star who spent a great deal of time in in La La Land!
Clearly El Lay was NOT a place he saw fit to raise a child!
As for who he left his entire estate to, it all went to his long-time girlfriend, Mimi O’Donnell. The two split just months before his apparent heroin overdose.

A few folks at FOX News have made their careers by crapping all over Hollywood, but this time they might of gone too far!
Earlier this month Sean Hannity and his panelists were discussing celebrities who died too young as the result of drug abuse, but the segment took a crazy turn when guests began predicting that Justin Bieber and/or Lindsay Lohan, for all their supposed substance troubles, might be the next ones to go!
Panelist Michelle Fields actually asserted that LiLo's mom Dina Lohan is "doing cocaine with her."
Believe it or not, the Lohan family's legal counsel didn't take kindly to those allegations! Nor did they appreciate any implication that Lindz might be susceptible to overdose!!
So, earlier today, Mark Heller & his legal eagles drafted up an articulate and super angry cease and desist letter and then sent it over to the head honchos at Fox!!!
We've obtained an EXCLUSIVE! copy of the fiercely fascinating letter, which you should read (below)!
what does the fox say what the FOX reporter said to get everyone so hot and bothered?
Ch-ch-check out the scandalous video…AFTER THE JUMP!!!
