The 21-year-old was diagnosed with kidney dysplasia when she was a child and most recently, received a life-saving kidney transplant from her father!

While posing for the cover of Seventeen magazine, the young actress opened up about her condition and revealed what she has learned from her experience. For starters, she realized the importance of family and said:

Sofia Vergara Launching Health And Wellness Site With Oscar De La Hoya

Sofia Vergara wasted no time to capitalize off of her Modern Family fame and she's not showing any signs of stopping!

The seksi Colombian actress is creating her own health and wellness website with world champion boxer Oscar De La Hoya!

The pair will join forces to launch SuperVida.com in 2012, a site catered to educate the Hispanic community about health issues, as Sofia explains:

“We are going to launch a website called SuperVida, dedicated to educating Hispanic families on health issues that most affect us. There are 50 million Hispanics in this country and there is not a resource like this and we need it. It is going to be revolutionary to better and save lives.”

Sofia says that being diagnosed and surviving thyroid cancer is what inspired her to create the site.
