After nearly four weeks, Philip Seymour Hoffman's cause of death has finally been confirmed.

It's even worse than expected.

The NYC medical examiner's office lists the cause of death as "acute mixed drug intoxication."

Philip's heroin addiction was well-known, but the coroner also found cocaine, benzodiazepines (an anti-anxiety medication), and amphetamines in his system.

The mix of heroin and cocaine, commonly referred to as a speedball, can be incredibly dangerous. John Belushi and Chris Farley also died with both drugs in their systems.

This lethal cocktail is officially what took Philip from us — and we all know he is still greatly missed.

Leslie Carter's Autopsy Comes Back Inconclusive, A Toxicology Report Is Next…

We're saddened to report that the Carter family will have to wait a bit longer to get closure.

Yesterday, we were heartbroken to learn that Nick and Aaron Carter's sister Leslie Carter passed away at the age of 25.

Then earlier today, we heard that Leslie has been battling an addiction to prescription medication, and that she had moved to Upstate New York in an effort to beat it.

Now, we've learned the upsetting news that Leslie's autopsy was inconclusive, and they'll have to perform a toxicology report due to the fact that there were no visible signs of trauma.

Here's what Coroner Richard Mackowiak had to say about it:

This just makes it all the more tragic.

Although the autopsy on Leslie Carter has yet to determine the cause of her sudden death, sources close to the family have revealed that the sister of Nick and Aaron Carter was battling an addiction to prescription medication, and had moved from Canada to Upstate New York in an attempt to beat it.

While the family members she was staying with had tried to make the environment as safe and sober for her as possible, they left her alone for an hour to run some errands, and when they returned, she was unconscious.

Heartbreaking. Especially to think she was trying to get herself help.

Our thoughts and prayers go out to her family and friends during this impossibly difficult time.

Kelly Osbourne Opens Up About Her Addiction To Painkillers! Took Up To 50 Pills A Day!

This is shocking.

People may forget - especially because of how lovely, well-spoken, and put-together she is now - that Kelly Osbourne struggled with a serious addiction to painkillers, but on her upcoming episode of Piers Morgan's Life Stories, she reveals just how severe it was!

She explains:

"At the start I only took one every weekend. Then it was one every weekday, then two a day, then three. By the end I was taking so many pills I was on my hands and knees vomiting blood because the drug was ripping away my stomach lining. I was taking 50 a day, more than twice the lethal dosage, and every day I took one more, hoping I wouldn't wake up. If I hadn't gone into rehab then, I'm sure I wouldn't be here now. I feel very lucky because people die every day from taking painkillers like Vicodin, and I was taking much more than them."

Unbelievable! And so terrifying!

Thank goodness she reached out for help and got treatment when she did! There's no way her body could have withstood that kind of dosage for much longer.

Addiction to prescription drugs is a serious problem that can spiral out of control very quickly, so we're so grateful that Kelly was able to recover and achieve what she has today!

We're so proud of her for being brave enough to speak so openly about something that must still be very difficult to look back on!

You've come a long way, bb! Congratulations!
