As you may know, on Sunday, tragedy struck when 10-year-old Caleb Schwab died while riding Verr├â┬╝ckt, a water slide billed as the tallest in the world, at Kansas City’s Schlitterbahn Water Park.

Following the accident, authorities were pretty tight lipped but PEOPLE learned that Caleb was actually decapitated.

A witness, Kelsey Friedrichsen, recalled making her way to the top of the slide around 3 p.m. when they saw a group gathering at the bottom. The shaken woman remembers:

“There was a woman being treated for some kind of injury on her face and head. That was when my boyfriend said, ‘I think there is someone else.'”

That’s when Friedrichsen’s boyfriend, Josh Foster, spotted the first responders and saw them cover the 10-year-old’s body. Josh also noticed that there were two women with the boy who were apparently not related to him but had minor injuries on their faces.

Kelsey continued:

“It looked like he must have somehow been ejected from his seat, bounced around between the netting and the slide and just slid down. He would have fallen down without the raft. It’s kind of like a tube.”

While we learned that the Verrückt was ruled unsafe back in 2014, before making some changes and re-opening, there was another witness that told ABC News that his 14-year-old cousin rode the slide earlier in the day and towards the end, the velcro on her seatbelt had come undone! YIKES!

We can’t begin to imagine what Caleb’s family is going through and our hearts go out to them during this extremely difficult time.
