Did a Bone Marrow Transplant Cure a Cancer Patient of AIDS?

A 42-year-old man who had both leukemia and AIDS received a bone marrow transplant — a common, late-stage treatment for that type of cancer. His doctor selected a bone marrow donor who had a rare genetic mutation that renders people virtually immune to HIV. The transplant appeared to cure the patient of AIDS.

We’re as wary as the next guy of inferring too much from a single case study. Maybe it was a fluke; maybe there were unknown factors at work. But this one is pretty intriguing.

The case was presented at a conference earlier this year (here’s the abstract), and written up in this morning’s WSJ.

As is common for bone marrow transplant recipients, the patient first had radiation and chemotherapy, which tends to kill off many of the immune cells that harbor HIV. After the transplant, the patient’s immune system was repopulated by cells created by the donor marrow.

The donor had a mutation, present in about 1% of Europeans, that creates immune system cells that lack a receptor molecule called CCR5. That receptor plays an important role in HIV’s ability to enter the cell. (Pfizer’s HIV drug Selzentry works by blocking CCR5.)

So the patient’s immune system was repopulated with immune cells that carried the mutation. And, nearly two years after undergoing the transplant, he shows no signs of having any HIV left in his body — despite the fact that he hasn’t taken any AIDS drugs since before the transplant.

Perhaps the most important caveat is just how risky bone marrow transplantation is: It’s given to cancer patients after other treatments fail, and it kills up to 30% of patients.

But researchers hope to apply the apparent lessons of this case to strategies using gene therapy (which carries its own risks) to try to induce the protective mutation in patients with HIV.

David Baltimore, who won a Nobel Prize for research on tumor viruses, has started a company to use gene therapy to target HIV. He calls this case “a very good sign” and a virtual “proof of principle” for gene-therapy approaches.

    June Brigham was the first case of a bone-marrow cure, back in 1990. Her identical twin was her donor, and it was done at the City of Hope in Duarte, CA. UNfortunately, her insurance company delayed the procedure to the point that she was too ill to survive long after the surgery. Her system, however, showed NO HIV at the time. Amazing.

    i believe that there may be a cure out there, just not available to the general public because its so expensive, i mean, how long has it been since Magic Johnson came out with HIV or AIDS, and he's still alive and kicking, and actually i read somewhere where it said that he was in"remission"., i didn't think that was possible, i wouldn't be surprised if he's cured, but has to keep it underwraps.

    Bone Marrow transplants may kill recipients 30% of the time, but full blown AIDS kills 100% of the time. It is worth the risk to investigate further if it will help eradicate this vicious disease. The challenge will not be the research but finding donors. The lives of many cancer patients are already lost because they cannot find a compatible donor, imagine trying to find donors with this gene mutation?

    Magic Johnson doesn't have full-blown AIDS, just HIV.
    His HIV hasn't developed into AIDS because of all the medicine he's taking.
    You could potentially have HIV your entire life and never get AIDS.

    Keep in mind that with regular drug treatments, most HIV carriers can live an average life span. Really and truely, a cure isn't what we need. A vaccination is.

Drugs whether the HIV cocktails or insulin for Diabetics or Blood Pressure Medication for Cardiac Patients are wonderful and prolong life BUT they have toxic side effects which over time are very debilitating. Although a drug regimen may control a specific disease, any doctor will tell you that drugs undoubtedly have over time deleterious effects other organs, for example, the kidneys, the cardiovascular system, the nervous system, etc. A cure is most definitely necessary in addition to a vaccine!

    Well but that is a catch: its hard to find a bone marrow compatible with another person. Its like a lotery. Most of the donaters are relative, or something like that. To find a compatible bone marrow randomly is almost impossible. So to cure billions of people that suffers from HIV by using a bone marrow that is only present in 1% of european population, will be impossible. Sadly, but true.

    It makes sense to me that it could; my son had leukemia and so we learned a lot about the blood disorders. AIDS, like leukemia, is in the blood cells.AIDS attacks the white-cells(for immunity). a BMT wipes out the entire immune system and all the blood cells; red-cells, platelets, white-cells….it kills off the cancer cells,and then the BM slowly recovers and rebuilds itself…free of the mutation(disease),and since AIDS is concentrated in the white-cells it makes sense a BMT can possibly "cure" it….

    Re: WawaLisa – A possibility(this is what we did) umbilical-cord stem cell banking; this saves a newborn's cord blood, which is compatable to ANY person; it is universal. It can be banked up to 10 years and given as a BMT. There is always a "match",and the good of it is, no one has to die for the stem cells to be donated; it's just in the cord(which they "milk" at the baby's birth) which they would throw away for disposal anyway.This new procedure has the capability of potentially saving many lives. Perhaps there could be a "bank" where everyone could donate their newborn's cord blood and have universal access to it?We banked our baby's blood and have it stored for our 10 year old, who is in remission for leukemia, in case, God forbid, he ever relapses and needs a BMT; it's sort of like insurance.

    Last semester for communications, my group was talking about safe sex. We had to argue to the class why we think safe sex is the best bet.
